WELLNESS COORDINATOR NURSE

Responsible for the assessment, monitoring and oversight of health care services to meet the needs of Community Integration and Habilitation Waiver consumers as per the service definition of Wellness Coordination Services.  Coordinates care with local healthcare providers to insure the health needs of consumers receiving Wellness Coordination.
